The following is an excerpt from a DD Form 1300 dated October 28, 1968.

CROWLEY, JAMES ALLEN II; 05 256 958; 1LT; USAR; ARMOR; AIR CAV TRP, 11TH ACR, VIETNAM

BATTLE CASUALTY

DIED 16 OCTOBER 1968 IN VIETNAM FROM WOUNDS RECEIVED WHILE PILOT OF MILITARY AIRCRAFT WHEN AIRCRAFT WAS HIT BY HOSTILE SMALL ARMS FIRE AND CRASHED.

COMMENCED TOUR IN VIETNAM 11 SEPTEMBER 1968


The following excerpt of an incident report regarding an OH-6A helicopter crash in Vietnam on October 16, 1968 was furnished by the Vietnam Helicopter Pilots Association.  1LT James A. Crowley II, a pilot with the 11th Armored Cavalry, died of injuries approximately 30 minutes after this crash.

This was a combat incident.  This helicopter was LOSS TO INVENTORY
This was a Recon mission for Unarmed Recon

Count of hits was not possible because the helicopter burned or exploded.
Small Arms/Automatic Weapons; Gun launched non-explosive ballistic projectiles less than 20mm in size (7.62MM)
This helicopter was hit in the Cockpit
Casualties = 01 INJ, 01 KIA ..
The helicopter crashed.  Aircraft Destroyed.
Both mission and flight capability were terminated.
VUNG TAU recovered the helicopter.
